Vial of Life

The Swansea Ambulance Corps strongly endorses the Vial of Life program. In a medical emergency one of our most important tools is information. The Vial of Life contains your important medical information and it will assist us in providing the proper medical treatment.

The form includes patient data such as name, address, phone, insurance info as well as medical information such as allergies, medications, and pertinent medical history. All of this information is optional. Fill out as much or as little as you wish. Place the completed form on your refrigerator. During a call to your home, we always check for this form.

Please also consider sending a copy of the completed Vial of Life to us by mail or fax. We can then have the information available to us as we respond to the call.
